While working at an apple orchard, Homer learns some powerfully indelible lessons about life, love, and home. The name "The Cider House Rules" refers to the list of rules that migrant workers are supposed to follow at the Ocean View Orchards. However, none of them can read, and they are completely unaware of the rules – which have been posted for years. After Arthur and his team return to the orchard the following season, Homer discovers that Rose, Arthur's daughter, is pregnant. He then makes the injury worse, and as a last request, asks Homer and another worker to tell the police his death was a suicide. He has taught Homer (Tobey Maguire), his protege, everything he knows about medicine, but Homer is opposed to abortion.

Michael Caine's performance is one of his best, and Charlize Theron is sweet and direct as the girl. He names the baby Homer Wells and essentially adopts the kid himself, teaching him everything he knows about medicine and grooming him to take over the institution. (If forged papers are necessary, no problem.) Homer, meanwhile, wonders if he might not be allowed to choose his own path in life. The Cider House Rules derives affecting drama from wonderful performances, lovely visuals, and an old-fashioned feel.

Wally and Candy are both impressed by Homer and his calm demeanor, and Wally asks Homer if he would like to drive back with them to Ocean View. While the visit is meant to be a short one, Dr. Larch and the orphans all understand that Homer will not be returning. Furious at Homer’s departure, Melony runs away from the orphanage to find him.
A subplot follows the character Melony, who grew up alongside Homer in the orphanage. After Homer leaves the orphanage, so does she in an effort to find him. She eventually becomes an electrician and takes a female lover, Lorna. Melony is stoic, who refuses to press charges against a man who brutally broke her nose and arm.
Many years later, teenaged Angel falls in love with Rose, the daughter of the head migrant worker at the apple orchard. Homer decides to return to the orphanage after Wilbur's death, to work as the new director. Though he maintains his distaste for abortions, he continues Dr. Larch's legacy of performing the procedure for those in need, and he dreams of the day when abortions are legal.
She tells Homer that if he can find a record of her mother’s identity in Dr. Larch’s office, she will perform sexual favors for him. Homer is caught snooping by Dr. Larch, who destroys all records of the mothers who drop off their children. Nevertheless, Homer and Melony begin a sexual relationship; Homer is not in love with Melony, but Melony is in love with him. As a baby and a young child, Homer Wells is adopted by several foster families. However, he is always returned to the orphanage and eventually comes to realize that his true home is there. Homer is both calm and a fast learner, and he becomes the unspoken favorite of Dr. Larch.

Homer and Candy become much closer during harvest and spend more time together while Wally is fighting in Burma. He implies that his daughter knifed him but also insists that his death be called a suicide. After the other harvest workers leave, Homer tells Angel the truth about his parentage; Candy, separately, tells Wally the truth.

Homer leaves the orphanage, and returns with them to Wally's family's orchard in Heart's Rock, near the Maine coast. Wally and Homer become best friends and Homer develops a secret love for Candy. Wally goes off to serve in the Second World War and his plane is shot down over Burma. He is declared missing by the military, but Homer and Candy both believe he is dead and move on with their lives, which includes beginning a romantic relationship. When Candy becomes pregnant, they go back to St. Cloud's Orphanage, where their son is born and named Angel. While Wally is away, Candy starts flirting with Homer, and they have an affair.
